Job: CASE MANAGER NURSE and DISEASE MANAGER NURSE

The Case Manager Nurse and Disease Manager Nurse are full-time positions. The Nurse Manager may qualify to work from a home office or onsite in the Tampa office. The Case/Disease Manager responsibilities include the collaboration and management of assigned cases to ensure costs are contained and quality of care is maintained as the patient accesses care and services in the continuum of care. The Care Manager performs telephonic reviews and compiles reports in accordance to client contractual requirements, using Milliman criteria, INETICARE approved MH/SA criteria and URAC/CMSA standards of care.

Registered Nurse with at least 5 years of Clinical Nursing Experience

Active, Unrestricted License

Certification Case Management, or a related field

Excellent oral, written, telephone, and computer skills

Positive, proactive, team-oriented approach/attitude

Familiar with URAC Standards, Principles, and Procedures
