Description:
Perform duties to coordinate and document client performance plans, transition plans and release plans for clients with mental health disorders.

Requirements:

Bachelor‘s Degree or equivalent in Social or Behavioral Science preferred. HIV/HOPWA knowledge, computer literate.

Must be able to work afternoon/evenings
